Your Journey Begins in Three Simple Steps

Step One — Submit a Referral

Referrals can be made by Support Coordinators, LACs, Plan Nominees, Families, or Participants directly.

What to provide:
  • Participant's support needs and goals
  • Assessments, if available
  • Preferred start date
  • NDIS plan details

Step Two — Eligibility & Needs Meeting

We meet with the participant, family, or SC to understand their individual needs. This meeting is gentle, trauma-informed, and respectful — designed to ensure a perfect fit, not judgement.

We will discuss:
Goals and support needs
Behavioural or sensory requirements
Preferred routines and medical needs
Community access requirements
Cultural considerations
STA or day program preference
Risk or safety planning
PBS strategies if required

Step Three — Transition & Start Date

We aim for smooth, stress-free transitions that feel safe, predictable, and supportive. Once approved:

Move-in plan is established
Participant meets their support team
Day program schedule is built
Medication & personal care routines confirmed
Household expectations explained in simple language
Family/SC receive regular communication
Participant begins their Safe Living journey confidently
